IMPORTANT PROGRAM NOTICES
2025 W-2s for CDPAP Personal Assistants
W‑2s will be mailed by January 31, 2026. Personal assistants can now access their W-2s electronically by logging in to PPL@Home and navigating to the “Tax Documents” tab at the top. Please allow until February 15 for mailed W‑2s to arrive. W‑2s will not be reissued before February 15.
2026 W-4 Forms for Personal Assistants
Personal assistants may notice that the 2026 IRS Form W-4 is now available in the Forms tab of PPL@Home. Currently, only the 2026 form is visible.
This does not impact your withholding calculation. The system is simply showing a blank 2026 form pre-populated with your address. Unless you update the 2026 form, your elections from your 2025 W-4 remain in place.
We will provide an update here when last year’s W-4 is available for viewing in PPL@Home. If you need a copy of your 2025 W-4 in the meantime, please contact PPL Customer Service at 1-833-247-5346.
Please note: You do not need to fill out the 2026 W-4 unless you claimed “Exempt” on your 2025 W-4.
CDPAP Training Update
Personal assistants (PAs) who began working in New York CDPAP in 2025 are required to complete training in the Nevvon portal by March 31, 2026. PAs who began working with PPL in 2026 must complete CDPAP training within 90 days of their PPL start date.
If you have not completed your training, please do this as soon as possible to remain compliant.

About
The Consumer Directed Personal Assistance Program (CDPAP) is a Medicaid program governed by Section 365-f of the New York State Social Services Law (SSL) and the implementing regulations under Section 505.28 of Title 18 of the New York Codes, Rules, and Regulations (NYCRR).
This program helps people with disabilities, chronic illnesses, or other long-term health conditions stay at home with their chosen care and services. They can employ people they choose as personal assistants (PAs).

All time entries, including paper timesheets, are due by 11:59 p.m. ET on Saturdays. Consumers/designated representatives (DRs) must approve PA hours by 12 p.m. (noon) ET on Sundays. Need help? Call 1‑833‑247‑5346 or visit the Payroll & Payment section of our PA Resources page.
View Payroll & Payment ResourcesService authorizations show the number of CDPAP hours a consumer is approved to use. PAs must be scheduled within these authorization limits to avoid payment issues, as payment can only be issued for the hours the consumer is authorized for in CDPAP. Consumers can view their full authorization details in the Service Authorizations tab of PPL@Home or quickly see how much has been used and is remaining in the Summary tab of PPL@Home.

Below are the steps to register with PPL as the statewide fiscal intermediary for CDPAP. If you need help, contact PPL at 1-833-247-5346, sign up for a virtual registration session or in-office appointment, or contact your CDPAP facilitator if you work with one.
Consumer Registration Steps
Step 1: Health Plan / LDSS Approval
Before registering with PPL, consumers must receive approval from a health plan or Local Department of Social Services (LDSS) and complete the required assessment. Consumers who have not had an assessment should contact the New York Independent Assessor Program (NYIAP) at 1-855-222-8350 (TTY 1-888-329-1541).
Step 2: Assessment & Approval
If approved for CDPAP, the health plan or LDSS will send a referral or authorization to PPL. Once PPL receives the authorization, we will contact the consumer to begin the registration process.
Step 3: Create a PPL@Home Account or Log in
To complete the registration process online, sign up for or create a PPL@Home account. This is where you will enter your information, sign the required Memorandum of Understanding (MOU), and view and manage information going forward.

To view and approve PA timesheets in PPL’s Time4Care™ app, download the app using the links below and log in with the same details you use for PPL@Home.

PA Registration Steps
Step 1: Consumer Association
Before you can register with PPL as a personal assistant in CDPAP, your consumer must be signed up with PPL, have a valid service authorization, and contact PPL to link you to their account.
Step 2: Create a PPL@Home Account or Log in
To complete the registration process online, sign up for or create a PPL@Home account.

- Offer Letter (one per associated consumer)

Step 3: Download Time4Care
If you have registered with PPL and your consumer has a valid service authorization, you can log your time in our Time4Care™ app. Download Time4Care using the links below and log in with the same details you use for PPL@Home. You can also complete your Electronic Visit Verification (EVV) training in Time4Care or PPL@Home.
